The following questionnaire will help you measure your general level of daytime sleepiness using the Epworth Sleepiness Scale. This is a validated test that can help you evaluate your level of daytime sleepiness. You are to rate the chance that you would doze off or fall asleep during the following situations (different, routine, daytime) using a scale from 0 (lowest chance) to 3 (highest chance):

People tell me I snore.

My bed-partner is disturbed by my snoring.

My snoring disturbs other in next room.

My snoring has become progressively worse.

I snore only when I sleep on my back.

I snore while sleeping in all positions.

I have been told I stop breathing between snores.

My snoring causes me to awaken suddenly.
